Deborah Smith Hoffman Mentor Award Nomination Form

Purpose:
To give recognition to a member of SCMEA who has served as a mentor to music educators in South Carolina in such a manner as to be deserving of this special award. This award is in honor and memory of Deborah Smith Hoffman and her contribution to music education in South Carolina.

Eligibility:
1. Must have served a minimum of five years in the music profession in South Carolina.
2. Must have been an active member of SCMEA for five years.
3. Must have demonstrated the highest ideals and professional integrity during time of service.

Application Process:
Complete and submit the online application. A letter of recommendation must be uploaded to the online application. This letter should demonstrate evidence of this teacher’s impact on music education in general. This letter should include the quality and diversity of this teacher’s impact as a mentor.

 

Selection Process:
The recipient of this award will be chosen by a committee with the SCMEA Immediate Past-President as chair and comprised of the current presidents of the SCMEA Divisions and the President of SCMEA.

Presentation:
The recipient of this award will be notified by January 15th. The award will be presented at the annual SCMEA Professional Development Conference at a time selected by the President and the Conference Committee. The award will consist of a plaque purchased by SCMEA and obtained by the Conference Chairperson. The SCMEA Public Relations Chairperson will publicize the award.
